Junior Master
Gardener Registration Agreement
The JMG Registration Agreement Form registers a properly organized and
functional JMG group. The registration serves as a permit for the use of
the 4-H and JMG service marks, names, logos and emblems. (PLEASE READ THE
“JUNIOR MASTER GARDENER PROGRAM POLICIES AND GUIDELINES".)
2.
Youth Group Enrollment Form
The JMG program reaches thousands of youth across the nation. To document
each youth’s participation in the program, the group leader must complete
the group enrollment form for each group. This information will provide
statistical data for sate and federal governments for future funding and
civil rights documentation.
Use the following guidelines when completing the enrollment form:
County: County in which the JMG
program is being conducted.
Group Name: The name can be selected or created by the group. If
another group
has already registered under the same name the group will be asked to
select
another name.
Delivery Mode: The JMG program can be organized in various
settings.
Organized 4-H Group: JMG group is part of an exiting 4-H program
such as a community club, project club or school club. (Those
participants who are 4-H members are considered duplicates in this
file.)
Special Interest: Organized or coordinated by Extension personnel
and directed and/or taught by volunteer leaders. The meetings are
conducted in informal classroom and/or garden settings.
School Enrichment: JMG curriculum is used in a classroom setting,
led by school faculty, a staff member or a volunteer. May be part of an
afterschool program, latchkey, or summer school.
3. Kansas 4-H
VIP (Volunteer Information Profile)
Individuals other than paid K-State Research and Extension personnel or
paid certified Kansas teachers are required to complete the 4-H Volunteer
Information Profile in order to become a Kansas Junior Master Gardener
Leader. K-State Research and Extension takes its obligations seriously to
provide a safe environment for all persons involved in youth activities.
This profile is designed to be an information gathering aid so as to
successfully match the individual’s skills and interests with the
appropriate volunteer position and includes the code of ethics for all
Kansas 4-H volunteers. Answers given by the individual are subject to
verification in those instances where a legitimate question arises as to
his/her qualifications. Please refer to the instructions on page 2 of the
profile.
THE VIP FORM WILL BE FORWARDED TO YOUR LOCAL COUNTY OR DISTRICT EXTENSION. |
