KSRE/Kansas 4-H Co-Brand Name Tags are available for order though the state 4-H office. Please email the attach form to kansas4h@ksu.edu or mail to:

Postage will be billed to the county if selected to be mailed. Name tags will be made within 7-10 business days. Make checks payable to Kansas 4-H Youth Development and mail to the above address. It will be up to the unit whether they would like it to read District or District, County or District, City, please indicate on the form how you would like it to read.
