4-H Participation Forms

4-H member safety is a top priority whether at county wide activities, club events or project meetings. Keep in mind that providing leaders with participation forms with emergency contacts and medical needs at their fingertips is an important risk management tool for insuring a safe environment. Participation forms should be printed for all participants, including volunteers working with 4-H youth, such as club leaders, project leaders, shooting sports coordinators and instructors.  Remember, completed participation forms are confidential; they need to be kept in a notebook or envelope, accessible only to the registered volunteers in charge.

Check out the revamped instructions and templates for printing participation forms using 4HOnline!  Staff can locate these resources on the KSRE 4-H Youth Development website, on the Staff Resources page for 4HOnline.  You will need to login with your eID to view.  Full instructions have been revised and “QuickTip” instructions been developed for staff who only need to brush up on the process.  The mail merge templates have also been updated to include fields for parent names and their contact information.  There is a 1 page and a 2 page template available, choose the version which best suits your needs.  To accommodate these changes, new Shared 4HOnline Participation Form Info reports have been created.  If you wish to use a new template, you must use a new report; your current Custom 4HOnline Participation Form reports will not contain all necessary fields.  Contact Terry Roberts at teresaroberts@ksu.edu or call 785-532-5800 for assistance.

 Just a friendly reminder, the state will print participation forms for events/trainings if registration is through Cvent.  KSRE staff are responsible for printing participation forms for local events.
